Registration

Registration for the 2026-2027 school year will open on July 7, 2026. Please check your email for more information on July 7. 

ALL PARENTS (INCLUDING INCOMING FRESHMEN) MUST COMPLETE THE ANNUAL REGISTRATION PROCESS.

Families need to follow the three-step process to complete registration to ensure a faster process. A computer is recommended to complete Online Registration. If you use a mobile device, you will need to use an internet browser.

STEP 1: Update Student Information

Parents and guardians are required to verify and update email addresses, phone numbers, emergency contacts, and health information through Skyward Family Access. Student information should be updated and completed for each student before August.

STEP 2: Pay Registration Fee Online

Pay before August 12 and SAVE $25 per student. Parents and guardians can pay registration fees online through Skyward Family Access. After logging in, go to “Fee Management” to pay the fees associated with your student.

STEP 3: Required Health Forms

Freshmen: Physical exam and dental exam Seniors: Meningococcal Vaccination **Please note that Athletic physicals do not meet the requirements for students entering 9th grade.

STEP 4: Distribution Day

All students must attend their designated Distribution Day. Wednesday, August 5: Seniors, Juniors, and Sophomores Thursday, August 6: Freshmen Distribution hours: 11:00 a.m. - 2:00 p.m. & 3:00 p.m. - 6:00 p.m.

Update Student Information

If you have a current D155 student you can use the same login information for Skyward Family Access. If you forgot your login or you’re new to D155, click forgot your login.

If you need assistance completing registration online or you do not have computer access, email your designated registrar listed above. 

Your student’s registration will not be complete until the forms have been submitted.

Pay Registration Fee Online via Skyward

$275 | PAY IN FULL BEFORE AUGUST 12 AND SAVE $25 PER STUDENT

$300 | Registration Fee (After August 11)

$250 | Driver Education

The registration fee includes unlimited participation in extracurriculars, a D155 student activity pass (non-state series), and a Chromebook.

Parents and guardians can pay registration fees online from July 7 to August 11 through Skyward Family Access. After logging in, go to "Fee Management" to pay the fees associated with your student. Discover, Visa, and MasterCard are accepted. All past-due fees/fines are due before Distribution Day.

Please note: Applications for Fee Waivers are available from Student Services after July 13.

FRESHMAN REQUIREMENTS

Freshman Physicals

All freshmen are required by law to complete a physical exam within 12 months of Freshman Distribution Day, August 6, 2026. Physicals conducted between August 7, 2025, and August 6, 2026, will satisfy this requirement.

The exam must be documented by a physician using the state-approved form available to download here, and the parent must complete the Health History section.

**Please note that Athletic physicals do not meet the requirements for students entering 9th grade.

 

Freshman Dental Exams

All freshmen are also required to submit proof of a dental exam within 18 months of the start of freshman year.

SENIOR REQUIREMENTS

Meningitis Vaccination

All seniors must meet the Illinois Department of Public Health immunization requirements for meningococcal disease prior to the start of 12th grade.

Proof of vaccination must be provided prior to Senior Distribution Day, August 5, 2026.

ALL STUDENTS

MEDICATION FORM

Any student needing to take medication (prescription and/or over-the-counter) must have a Medication Form on file with the school nurse. Forms must be submitted annually. The form is available to download here.

ASTHMA ACTION PLAN

Students who have asthma are required to have an "Asthma Action Plan." It must be kept on file in the school nurse's office. The form is available to download here.

Distribution Day

All students must attend their designated distribution day listed below. Students will receive their class schedule and textbooks and have their student ID and yearbook pictures taken. Students will also be required to turn in a physical, vaccination record, and/or medication form. 

Please plan accordingly; the first 30 minutes of each session is usually quite busy.

Wednesday, August 5: Seniors, Juniors, and Sophomores

Thursday, August 6: Freshmen

Distribution hours:  11:00 a.m. - 2:00 p.m. and 3:00 p.m. - 6:00 p.m.
